What is required?

1. Pharmacy Licensing & Registration

  • Current, valid state pharmacy license

  • Copy of DEA registration (if applicable)

  • Proof of business registration (state or federal)

  • Physical pharmacy address verification (utility bill or lease agreement)

  • Owner or corporate officer identification (driver’s license or state ID)

4. Compliance & Verification Materials

  • Compliance officer or contact person details

  • Written compliance plan or internal review checklist

  • Record of any disciplinary actions or board inquiries in the last 5 years

  • Website domain ownership verification (matching business name)

  • Digital security statement (SSL certificate, data protection, and privacy practices)

7. Supporting Documentation

  • Most recent state inspection report or internal audit summary

  • Sample labels or prescription packaging example

  • Return, refund, and complaint policy

  • Description of inventory management system

  • Patient safety or adverse event reporting procedure

2. Professional Credentials

  • Pharmacist-in-charge (PIC) name and license number

  • Professional liability insurance or business insurance policy

  • State board inspection report (if available)

  • Any existing accreditations or certifications (e.g., LegitScript, NABP, URAC, etc.)

5. Payment & Merchant Processing Requirements

  • Current merchant processing statement (if applicable)

  • Bank letter verifying business account

  • Card-not-present transaction policy or terms of sale

  • Refund and cancellation policy (posted online if applicable)

  • AVS (Address Verification System) and CVV policy for online transactions

  • Proof of PCI DSS compliance (if processing online payments)

8. Attestations & Agreements

  • Signed ScriptSafe Accreditation Agreement

  • Signed Attestation of Truth and Accuracy for all documentation

  • Consent for ScriptSafe to conduct verification checks and background review

3. Business Operations Documentation

  • Articles of Incorporation or business formation documents

  • Federal Tax ID (EIN) confirmation letter

  • Pharmacy hours of operation and contact details

  • List of pharmacy services offered (e.g., compounding, retail, mail-order)

  • Policies & procedures manual (including privacy, HIPAA, and patient safety policies)

6. Digital & Website Information

  • Pharmacy website URL (must list physical address and contact number)

  • Screenshots or documentation of:

    • Prescription verification process

    • Order flow and patient data protection

    • “About” and “Contact” pages showing transparency

  • Domain registration data (WHOIS record or registrar statement)

  • Disclosure of third-party platforms or telehealth partners (if used)

  • Certificate of Insurance (COI) for cybersecurity or data breach coverage

  • Marketing materials or website disclaimers (showing compliance language)

  • Credit card processor letter of good standing (if already onboarded)

After submitting their application and required documentation, our team conducts an initial review within 2–3 business days to verify all requested information has been submitted.
