Requirements to Apply

Current, valid state pharmacy license
Copy of DEA registration (if applicable)
Owner or corporate officer identification (driver’s license or state ID)
NPI Number

Pharmacist-in-charge (PIC) name and license number
Any existing accreditations or certifications (e.g., LegitScript, NABP, URAC, etc.)

Articles of Incorporation or business formation documents
Federal Tax ID (EIN)
List of pharmacy services offered (e.g., compounding, retail, mail-order)
Business model description
Owner/principal information (name, DOB, address, ownership %, ID upload, identity verification)
Business type (Storefront, Mail Order, Internet, Telephone, Closed-Door, Distributor)
Business practices questionnaire (controlled substances, compounding, 340B, etc.)

Record of any disciplinary actions or board inquiries in the last 5 years

Accreditation Timeline
