Requirements to Apply

Current, valid state pharmacy license
Copy of DEA registration (if applicable)
Proof of business registration (state or federal)
Physical pharmacy address verification (utility bill or lease agreement)
Owner or corporate officer identification (driver’s license or state ID)

Pharmacist-in-charge (PIC) name and license number
Professional liability insurance or business insurance policy
State board inspection report (if available)
Any existing accreditations or certifications (e.g., LegitScript, NABP, URAC, etc.)

Articles of Incorporation or business formation documents
Federal Tax ID (EIN) confirmation letter
Pharmacy hours of operation and contact details
List of pharmacy services offered (e.g., compounding, retail, mail-order)
Policies & procedures manual (including privacy, HIPAA, and patient safety policies)

Compliance officer or contact person details
Written compliance plan or internal review checklist
Record of any disciplinary actions or board inquiries in the last 5 years
Website domain ownership verification (matching business name)
Digital security statement (SSL certificate, data protection, and privacy practices)

Bank letter verifying business account
Card-not-present transaction policy or terms of sale
Refund and cancellation policy (posted online if applicable)
AVS (Address Verification System) and CVV policy for online transactions
